MDS Coordinator Job Description
Position Summary:
The MDS Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) assessments to ensure accurate and timely submission in accordance with federal and state regulations. This position plays a key role in developing individualized care plans, monitoring resident progress, and ensuring the accuracy of clinical documentation that supports quality care and reimbursement compliance.
Essential Duties and Responsibilities:
Assessment and Documentation:
- Coordinate, schedule, and complete MDS assessments for all residents per CMS and state guidelines.
- Ensure timely and accurate submission of MDS assessments to the Quality Improvement and Evaluation System (QIES).
- Review resident records, progress notes, and assessments to ensure all documentation supports MDS accuracy.
- Maintain up-to-date knowledge of RAI (Resident Assessment Instrument) guidelines and ensure compliance with all documentation standards.
- Collaborate with interdisciplinary team members to ensure that assessments reflect accurate clinical and functional status.
Care Planning:
- Lead and coordinate interdisciplinary Care Plan Meetings and ensure care plans are person-centered, accurate, and reflective of residents’ current needs and goals.
- Monitor and update care plans as needed based on resident condition changes or clinical updates.
- Provide guidance to nursing and support staff regarding documentation practices that impact care planning and MDS coding.
Quality and Compliance:
- Audit and monitor medical records to ensure completion and accuracy of supporting documentation for MDS items.
- Ensure compliance with state and federal regulations regarding resident assessments and reporting.
- Participate in quality improvement initiatives and assist in developing corrective actions for survey deficiencies related to documentation or assessments.
- Provide staff education and training on MDS procedures, documentation accuracy, and care planning best practices.
Communication and Collaboration:
- Communicate regularly with the Director of Nursing, Administrator, and interdisciplinary team regarding MDS scheduling, updates, and outcomes.
- Act as a liaison between nursing, therapy, dietary, and social services to ensure accurate input and coordination of resident data.
- Maintain open communication with external agencies and auditors when required.
Qualifications:
- Current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of employment (RN preferred).
- Minimum of 2 years of clinical experience in long-term care or skilled nursing facility.
- Prior MDS 3.0 assessment experience required.
- Strong understanding of RUG-IV, PDPM, and RAI processes.
- Proficient in EMR systems and MDS submission software.
- Excellent organizational, analytical, and communication skills.
- Ability to manage multiple priorities with accuracy and attention to detail.
Physical Requirements:
- Ability to sit, stand, and walk for extended periods.
- May be required to lift, push, or pull up to 25 pounds.
- Must be able to handle periods of high stress and deadlines.
Work Schedule:
- Full-time position, typically Monday through Friday.
- Flexibility required for meetings, training, or deadlines.
Pay: $32.00 - $35.00 per hour
