Job Summary:
We are seeking a dedicated and detail-oriented Resident Care Coordinator to oversee the delivery of comprehensive care within our senior living community. The ideal candidate will possess extensive clinical knowledge, exceptional organizational skills, and a passion for improving resident outcomes. As a Resident Care Coordinator, you will coordinate multidisciplinary care plans, supervise staff, ensure regulatory compliance, and promote a compassionate environment that prioritizes resident safety and satisfaction.

Responsibilities of the Resident Care Coordinator:
- Exhibits leadership qualities and communicates professionally with employees, residents, family members, responsible parties, and other healthcare professionals.
- Implements and updates care and services provided to residents through individualized service plans and communication logs.
- Coordinates, monitors, and assists with activities of daily living in alignment with a person‑centered care philosophy.
- Documents and reports changes and observations related to resident needs and preferences.
- Participates in quarterly service plan meetings.
- Communicates with the health services team, physicians, other healthcare providers, and responsible parties as needed.
- Addresses resident, family, and responsible party concerns in a timely and professional manner.
- Recruits and coordinates training for new Care Partners and Med Techs.
- Manages Care Partner and Med Tech schedules to ensure adequate staffing coverage.
- Fills in as needed to cover open shifts.
- Assists with daily tasks and requirements as requested.
- Ensures residents are treated with privacy, respect, and dignity.
- Follows standard precautions and infection control procedures.
- Completes state‑required training upon hire and annually.
- Participates in new employee orientation and monthly in‑services.
Required Experience:
- High school diploma or equivalent.
- A minimum of two (2) years of prior senior living experience is strongly desired.
- Associate degree preferred; CNA and/or Caregiver experience is a plus.
- Must be at least 18 years of age.
- Basic computer skills.
- Must be able to speak, read, and write in English.
