A well‑established senior living community in the Springfield, OR area is seeking a passionate and experienced Licensed RN to serve as our Health and Wellness Director. This leadership role oversees our Health Services program and ensures high‑quality care is provided to the residents in our community.

Responsibilities of the Health and Wellness Director:
- Exhibits leadership qualities and communicates professionally with employees, residents, families/responsible parties, and other healthcare professionals.
- Oversees the overall supervision, provision, and quality of nursing and supportive services for the community.
- Performs resident assessments and identifies needs related to physiological, psychological, environmental, and social factors upon move‑in and when changes of condition occur.
- Ensures routine quarterly assessments are completed.
- Ensures resident safety needs are met using appropriate assessment and evaluation tools.
- Oversees and participates in the development of care plans.
- Ensures communication regarding resident status is appropriately relayed to families/responsible parties, physicians, leadership, and state agencies when required.
- Investigates incidents and behavior changes requiring licensed nurse assessment and documents all findings in resident progress notes.
- Performs medication room and medication cart audits as needed.
- Monitors the 24‑hour communication and alert system for compliance and assesses residents placed on alert status.
- Reviews physician transcriptions, progress notes, discharge orders, move‑in orders, and hospital documentation.
- Assists leadership in ensuring policies, procedures, and state regulations are followed.
- Hires, trains, and evaluates health services employees, ensuring all direct care staff meet state‑required training standards.
- Participates in new employee orientation and monthly in‑services.
- Responsible for staff scheduling, supply purchasing, budget forecasting, and compliance.
- Ensures residents are treated with privacy, respect, and dignity.
- Follows standard precautions and infection control procedures.
- Completes state‑required training upon hire and annually.
Required Experience:
- High school diploma or equivalent.
- Current RN license in the state of Oregon.
- One year of prior nursing experience in a senior living environment preferred; Memory Care experience strongly valued.
- Basic computer skills.
- Must be able to speak, read, and write in English.
- Ability to obtain continuing education credits as required by state regulations.
Job Type:
