Responsibilities:
- Provide personal care assistance including bathing, grooming, dressing, and oral hygiene for hospice patients
- Assist with mobility, transfers, and repositioning to ensure patient comfort and safety
- Monitor and report changes in patient condition to the supervising nurse or care team
- Offer emotional support and companionship to patients and their families
- Assist with light housekeeping, meal preparation, and maintaining a clean care environment
- Document care activities accurately and in accordance with hospice care plans
- Collaborate with interdisciplinary hospice team members to deliver coordinated, patient-centered care
Requirements:
- High school diploma or GED required
- Current Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred
- Prior experience in hospice, palliative care, or home health care is required
- Strong interpersonal skills with a compassionate, patient-centered approach
- Ability to work independently and manage time effectively in a home care setting
- Valid driver's license and reliable transportation required
- Ability to pass a background check and meet state compliance requirements
