Who You Are as a Spiritual Care Coordinator

We are seeking someone who has:
- Experience in spiritual care, pastoral care, ministry, chaplaincy, hospice, home health, or a related setting
- A compassionate and patient-centered approach
- The ability to provide support to patients and families from diverse backgrounds and belief systems
- Strong communication and documentation skills
- The ability to work independently while collaborating with an interdisciplinary team
- A valid driver's license and reliable transportation for travel within the service area
What You'll Do
- Provide spiritual support and counseling to patients, families, and caregivers
- Assess spiritual needs and help incorporate them into the patient's overall plan of care
- Offer comfort, guidance, and emotional support during illness, decline, grief, and end-of-life care
- Collaborate with nurses, social workers, aides, and other members of the interdisciplinary team
- Participate in care planning and team meetings
