Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. The Good Samaritan Hospice team works together to provide relief through pain control and symptom management, while offering emotional, social and spiritual support for the patient and family. Good Samaritan Hospice is looking to hire an Intake LPN Coordinator to support our team in accepting patient referrals from hospitals, health care facilities, and physicians for hospice care. The Intake LPN assists the caller in understanding the hospice services that can be provided through the agency. This is a full time position working Monday through Friday with a weekend rotation (every third), and is based out of the Valencia Office.

QUALIFICATIONS for the Intake LPN Coordinator position:
- Licensed practical nurse with a current valid Pennsylvania license
- Minimum of two [2] years of current experience as a registered nurse or licensed practical nurse in health care and one [1] year of home health experience preferred
- Knowledge base of agency policies, federal and state regulations and third-party payers
- Skilled in critical thinking, communication, negotiation and collaboration
