Responsibilities:
- Provide personalized care to clients according to their individual care plans
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting
- Assist with mobility, transfers, and range-of-motion exercises to promote physical well-being
- Prepare nutritious meals and assist with feeding, taking into account dietary restrictions and preferences
- Document care provided and report any changes in the client's condition to the supervisor
- Collaborate with the care team to ensure coordinated and holistic care for each client
- High school diploma or equivalent
- Current CNA license
- CPR certification
- Compassionate and patient demeanor with excellent communication skills
- Reliable, punctual, and responsible
- Ability to follow instructions and work independently, as well as part of a team
- Physical ability to assist with lifting, bending, and providing personal care
- Valid driver’s license and reliable transportation (if required for client transportation)
