At Comprehensive Home Health Solutions, you'll play a pivotal role in enhancing individuals' independence and overall well-being in the familiar surroundings of their homes.
Responsibilities:
- Provide personalized care to clients in accordance with their individual care plans
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting
- Assist with mobility, transfers, and range of motion exercises to promote physical well-being
- Prepare nutritious meals and assist with feeding, considering dietary restrictions and preferences
- Maintain a clean and safe environment for clients by performing light housekeeping duties
- Provide companionship and emotional support, engaging in meaningful conversations and activities
- Accompany clients to medical appointments, shopping, and other outings as needed
- Document care provided and report any changes in client's condition to the supervisor
- Collaborate with the care team to ensure coordinated and holistic care for each client
- High school diploma or equivalent
- Compassionate and patient demeanor with excellent communication skills
- Reliable and punctual with a strong sense of responsibility
- Ability to follow instructions and work independently as well as part of a team
- Physical ability to assist with lifting, bending, and providing personal care
- Valid driver's license and reliable transportation (if required for client transportation)
