In this role, you will gain expertise in dealing with the complex medical, psychosocial, and emotional needs of patients while fostering compassion and empathy. The position offers exposure to a wide variety of traditional social work responsibilities, including but not limited to:
- Conducting comprehensive psychosocial assessments to evaluate residents’ needs and support systems
- Developing and contributing to individualized care plans
- Facilitating resident and family meetings to discuss care goals and concerns
- Implementing and supporting behavioral interventions
- Coordinating discharge planning and transitions of care
- Collaborating with an interdisciplinary team including nursing, therapy, and medical staff
- Providing emotional support and resources to residents and their families
- Assisting with regulatory compliance, auditing, and survey preparation
- Performing additional duties as assigned by the Director of Social Services or Administrator
- Must have LSW or CSW
- Experience in a long-term care setting as a Social Worker is preferred
- Ability to work independently or part of a group.
- Computer knowledge and experience using Point Click Care (PCC) preferred
- Ability to build and maintain rapport with residents and peers.
