Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, noting vital signs and changes in condition to ensure safety and well-being
- Provide medication reminders and assist with medication administration following care plans and HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with light housekeeping tasks such as cleaning and laundry
- Use specialized equipment like Hoyer lifts or other assistive devices to safely transfer clients with mobility challenges
- Support clients in engaging with their community through social activities or outings, fostering independence and social interaction
- Maintain detailed documentation of care provided, patient observations, and any incidents in compliance with privacy standards
Requirements
- Proven experience in home & community care, assisted living, nursing homes, or group home settings
- Knowledge of patient observation techniques, vital signs monitoring, and medication administration
- Certifications such as CPR, First Aid, and HIPAA compliance are highly preferred
- Experience working with individuals with developmental disabilities, Alzheimer’s disease, dementia, autism or other special needs is advantageous
- Ability to perform heavy lifting when necessary and operate assistive devices safely
- Strong communication skills to collaborate effectively with clients, families, and healthcare teams
