Community Physicians is seeking compassionate, detail-oriented, and experienced RN Case Managers/ Licensed Social Workers to join our dynamic case management team. This role focuses on facilitating safe and effective discharge plans for hospitalized patients by assessing their clinical, psychosocial, and post-acute care needs.
The ideal candidate will work closely with our physicians, nurses, therapists, and external partners to ensure continuity of care and minimize readmissions. This position combines clinical assessment skills with strong knowledge of community resources, payer guidelines, and transitional care planning. You will act as both an advocate for the patient and a steward of efficient resource utilization within the hospital system.
Qualifications:
- Education: Bachelor’s or Master’s degree in Social Work (BSW/MSW), Nursing (BSN), or related healthcare field required.
Licensure/Certification:
- Current state licensure (ex. LSW, LCSW, RN) as applicable.
Experience: Minimum of [1–2] years in hospital discharge planning, case management, or related field preferred.
- Familiarity with community resources, payer systems, and regulatory guidelines.
Skills:
- Strong assessment, critical thinking, and problem-solving skills.
- Excellent interpersonal, communication, and organizational abilities.
- Proficiency in EMR systems and documentation.
Responsibilities
- Collaborate with physicians, nursing staff, and other interdisciplinary team members to develop individualized discharge plans
- Conduct comprehensive psychosocial assessments and discharge planning evaluations
- Identify and address social, emotional, and financial barriers to discharge
- Arrange post-acute care services (ex. home health, skilled nursing facilities, rehabilitation, hospice).
- Provide counseling, crisis intervention, and resource support to patients and families
- Coordinate referrals to community agencies and services.
- Ensure compliance with hospital policies, federal and state regulations, and payer requirements.
- Document all case management and discharge planning activities accurately and timely in the EMR.
