Discharge Planner
Location: BryLin Hospital, Upstate New York
Schedule: Full-time, 8:00 AM - 4:00 PM (no weekend requirements!)

Position Summary:
As a Discharge Planner, you will play a vital role in bridging the gap between hospital care and post-discharge needs. You will work directly with patients, families, and the interdisciplinary team to ensure a smooth transition to outpatient services or other support systems.
Key Responsibilities:
- Provide pre- and post-discharge information, assistance, and referrals to patients and families.
- Collaborate with the interdisciplinary team to coordinate discharge activities.
- Facilitate adult, child, and adolescent group therapy sessions.
- Assess patients' needs, including housing, transportation, and continuing care, and make appropriate referrals.
- Notify outpatient providers of hospitalization with patient consent.
- Maintain detailed and timely documentation of discharge plans and related processes.
- Develop and evaluate educational materials for patients and families.
- Participate in clinical supervision sessions and team meetings.
- Support Utilization Review efforts to optimize patient care and insurance benefits.
Qualifications and Skills:
- Bachelors degree in related field required
- Strong written and verbal communication skills with the ability to adapt to diverse audiences.
