Position: Nurse Case Manager (Hybrid)
Location: Oakbrook Terrace, IL Hybrid Remote)
Schedule: Monday to Friday, 8-hour shift
Type: Full-Time
Pay: $ $36.00 per hour
About the Role:
We are seeking a compassionate and experienced Nurse Case Manager to join our team. In this role, you will serve as a key advocate and care coordinator, developing and managing comprehensive care plans to promote the health and well-being of our patients. This hybrid position allows for a blend of on-site and remote work.
Key Responsibilities:
- Develop, implement, and revise individualized patient care plans in collaboration with patients, families, physicians, and healthcare teams.
- Act as the primary point of contact and advocate for patients and their families, facilitating access to medical providers and community resources.
- Provide essential health education to patients and families to improve understanding and management of chronic conditions.
- Conduct thorough assessments to identify unmet health and social needs.
- Ensure medication adherence and compliance with follow-up appointments through ongoing support and communication.
- Maintain timely and accurate documentation and communicate effectively with all members of the care team regarding patient status.
Qualifications:
- Required:
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Illinois.
- Minimum of 3 years of nursing experience.
- Minimum of 2 years of experience with geriatric patients.
- Ability to commute to and work on-site in Oakbrook Terrace, IL, as needed for a hybrid schedule.
- Preferred:
- 2 years of experience in Case Management or Care Coordination.
- Experience in post-acute or long-term care settings.
