  • Support additional tasks as assigned by the Program Director.
  • Document mentoring sessions, activities, and participant progress toward individualized goals.
  • Assist with the development of Individualized Service Plans (ISP) and Safety/Crisis Plans, ensuring youth voices and needs are represented.
  • Maintain accurate and up-to-date case records of participant activities and outcomes.
  • Monitor and assess participant engagement, responsiveness to mentoring, and progress toward positive behavior change.
  • Adhere to CMS policies and procedures in all service delivery.
  • Model professionalism and the founding principles of CMS in all interactions with youth, families, and community partners.
  • Increase community awareness of the AIM Program by participating in outreach, networking, and community activities with youth.

  • Participate in unit initiatives and agency-wide committees, representing youth perspectives.
  • Partner with Parent Coaches and other CMS programs to provide holistic support for participants.
  • Meet regularly with the program supervisor for supervision, case consultation, and professional development.
