Focus on triaging calls and in basket messages, patient education, assessments, implementing and monitoring care plans and ensuring Quality outcomes.

Qualifications
Graduate of accredited school of Nursing, BSN strongly preferred, Demonstrated proficiency with EHR, with EPIC a strong plus.. The ideal candidate will also have worked in an outpatient setting and experience working with underserved populations.
