The Community Health Center of Cape Cod offers a rewarding, team centered environment. You will be responsible for the coordination of services and pursuit of excellence in integrated care for our patients. Focus on triaging calls and in basket messages, patient education, assessments, implementing and monitoring care plans and ensuring Quality outcomes.

Onsite position Monday- Thursday 8am to 6pm
Qualifications:Graduate of accredited school of Nursing, BSN strongly preferred, Demonstrated proficiency with EHR, with EPIC a strong plus.
The ideal candidate will also have worked in an outpatient setting and experience working with underserved populations.
Salary : $39 - $42
