ACT Nurse

Purpose:

The ACT Nurse provides comprehensive case management and nursing services to C4 clients diagnosed with medical disorders and severe mental illness. 

Minimum Qualifications:

1. Current license as a Registered Nurse in Illinois 
2. Bachelor’s degree 
3. Minimum of 3  years of experience in a hospital or clinic setting as a registered nurse
4. Valid Basic Life Support (BLS) certification
5. Personal vehicle, valid Illinois driver’s license, and insurable driving record required

Competencies (Minimum Skills, Knowledge, and Experience):

1. Ability to provide routine nursing care to clients, including administration of STAT oral and injectable psychotropic medications, evaluation of physical condition, and measurement of vital signs
2. Knowledge of infection control principles, and infection control policies and procedures that meet Joint Commission, OSHA, and CDC standards, as well as other federal, state, and local standards pertaining to infection control
3. Knowledge of behavior management techniques
4. Knowledge of HIPAA privacy and security regulations
5. Demonstrated presentation skills
6. Experience in training clinical staff
7. Communication skills to include interpersonal, teamwork, and collaboration skills.
8. Ability to be respectful of the diverse cultures of the people served and to provide culturally appropriate, competent and individualized treatment according to each client's age, gender, race, ethnicity, and cultures

Duties and Responsibilities 

1. Clinical Duties
   1. Provides routine nursing care to clients including, but not limited to, administration of STAT oral and injectable psychotropic medication, evaluation of physical condition, and measurement of vital signs
   2. Facilitates supportive treatment intervention to maintain or enhance the client’s capacity for independent functioning such as phone consultation with pharmacies and prior authorization with third party insurers 
   3. Interviews and evaluates clients to identify medical issues requiring  urgent attention, such as hospitalization,  or non-urgent follow up 
   4. Provides medical referrals to clients and their families
   5. Assesses clients for indications of abuse
   6. Responds to requests for emergency medical care of clients within the facility
   7. Records and documents diagnostic evaluations and treatment following established agency policies and procedures
   8. Provides coverage to maintain clinical services as needed
   9. Works collaboratively with C4 staff, on call psychiatrist and on call medical MD
   10. Other duties as assigned by supervisor
