Purpose: 

As a member of the Community Support Team (CST), the Case Manager will coordinate and provide comprehensive recovery and rehabilitation services to consumers participating in CST services. Interventions include a range of therapeutic and behavioral services for individuals with both severe mental illnesses and/or substance abuse disorders. Services are general delivered in the community. The Staff Therapist will provide services in a culturally sensitive and appropriate manner.


Duties and Responsibilities:


  1. Provide case management, outreach, and aftercare services
  2. Assess clients functioning, strengths, needs, and diagnosis
  3. Assess for indicators of abuse
  4. Develop treatment plan collaboratively with client by defining goals, specific outcomes, and time frames for treatment
  5. Apply appropriate behavior management techniques
  6. Provide culturally appropriate, competent and individualized linkage plans in accordance with each clients age, gender and gender identity, sexual orientation, race, ethnicity, religious beliefs and culture
  7. Determine appropriate type and level of services utilizing the Level of Care Utilization System for Psychiatric and Addiction Services (LOCUS) developed by the American Association of Community Psychiatrist
  8. Facilitate psychiatric hospitalization when indicated
  9. As appropriate, develop a mental health assessment or admission note resulting in the identification of the individual's mental health service needs and recommendations for service delivery
  10. Work cooperatively with other members of the treatment team 
  11. Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the community
  12. Assist with outcome measures
  13. Maintain compliance at all times with clinical documentation, timelines and agency policies and procedures including meeting required timeframes for documentation and service notes
  14. Report to assigned supervisor and actively seek consultation whenever necessary or requested by supervisor 
  15. Develop close working relationships with DCFS and DMH funded agencies, and other mental health service providers throughout the north and west sides of Chicago and suburbs
  16. Maintain compliance with confidentiality policies and procedures at all times
  17. Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the community 
  18. Other duties as assigned by supervisor


Minimum Qualifications:

  1. Bachelors Degree in Social Work, Psychology, or a related field is required
  2. Knowledge of Recovery principles, Co-occurring Disorders Treatment concepts, and/or Evidence Based Treatment Models preferred 
  3. Excellent interpersonal, verbal and written communication skills.
  4. Personal vehicle, valid Illinois drivers license, insurable driving record, and vehicle insurance is required.
