Purpose:

Members of the Assertive Community Treatment (ACT) Team will coordinate and provide comprehensive recovery and rehabilitative services to mental health consumers participating in ACT. Interventions include a range of therapeutic and behavioral services, including crisis intervention, for individuals with both severe mental illnesses and/or substance abuse disorders. Services are predominantly community based, including at consumers' homes, entitlement agencies, medical facilities, and other locations as needed. ACT services are available to consumers on a 24 hour/7 day per week basis, requiring occasional off-hour crisis response, evening and weekend coverage as needed, and participation on the ACT Team's 24/7 on-call schedule as assigned by the ACT Team Supervisor.

Duties and Responsibilities:

  • Provide case management, outreach, and aftercare services
  • Assess clients functioning, strengths, needs, and diagnosis
  • Assess for indicators of abuse
  • Develop treatment plan collaboratively with client by defining goals, specific outcomes, and time frames for treatment
  • Apply appropriate behavior management techniques
  • Provide culturally appropriate, competent and individualized linkage plans in accordance with each clients age, gender and gender identity, sexual orientation, race, ethnicity, religious beliefs and culture
  • Determine appropriate type and level of services utilizing the Level of Care Utilization System for Psychiatric and Addiction Services (LOCUS) developed by the American Association of Community Psychiatrist
  • Facilitate psychiatric hospitalization when indicated
  • As appropriate, develop amental health assessment or admission note resulting in the identification of the individual's mental health service needs and recommendations for service delivery
  • Work cooperatively with other members of the treatment team
  • Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the community
  • Assist with outcome measures
  • Maintain compliance at all times with clinical documentation, timelines and agency policies and procedures including meeting required timeframes for documentation and service notes
  • Report to assigned supervisor and actively seek consultation whenever necessary or requested by supervisor
  • Develop close working relationships with DCFS and DMH funded agencies, and other mental health service providers throughout the north and west sides of Chicago and suburbs
  • Maintain compliance with confidentiality policies and procedures at all times
  • Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the community
  • Other duties as assigned by supervisor

Minimum Qualifications:

  • Bachelors Degree in Social Work, Psychology, or a related field is required
  • Knowledge of Recovery principles, Co-occurring Disorders Treatment concepts, and/or Evidence Based Treatment Models preferred
  • Excellent interpersonal, verbal and written communication skills.
  • Personal vehicle, valid Illinois drivers license, insurable driving record, and vehicle insurance is required.

Salary : $45,000
