CapitalCare Family Practice Averill Park is looking for a part-time Nurse
Practitioner or Physician Assistant to join our friendly, close-knit primary care team. As part of Community Care Physicians (CCP)—the Capital Region’s first and largest independent multispecialty medical group—we combine the warmth of a hometown practice with the resources of a well-established organization.

Position Overview
As an Advanced Practitioner at CapitalCare Family Practice Averill Park, you’ll provide comprehensive family medicine care to patients of all ages. You’ll work alongside experienced physicians, nurses, and support staff who truly value teamwork, collaboration, and patient-centered care.
You’ll be encouraged to practice to the full extent of your license, supported by CCP’s integrated network of specialists and resources—while still enjoying the personal connections that make community-based medicine so rewarding.
Responsibilities
- Provide compassionate, high-quality care for acute, chronic, and preventive health needs
- Perform patient assessments, establish diagnoses, and develop individualized treatment plans
- Order, interpret, and follow up on labs and diagnostic studies
- Prescribe medications and manage ongoing conditions
- Educate and counsel patients and families on wellness, prevention, and healthy lifestyles
- Collaborate closely with physicians and care team members to ensure continuity of care
- Build meaningful, long-term relationships with patients in the Averill Park community
