Work consistently with the IDD population, focusing on empowering them to live with dignity and respect. Our model offers decreased driving time compared to traditional home care, typically staying within a 15-mile radius. CBNS currently serves Stark, Summit, Medina, Cuyahoga, and Lorain counties and is continuing to grow!
Position Schedule
This is a set schedule for our West Akron:
- Tuesday & Thursday - 3 pm - 9:30 pm
- Every other Friday, Saturday, & Sunday (3/6-3/8): 5:30 AM - 10:20 AM & 4:30 PM - 9:30 PM
Compensation
The estimated bi-weekly gross pay for this position ranges from $2076 - $2442

Role and Responsibilities
As a nurse on our Brookshire Run, you will primarily complete scheduled skilled nursing visits within group home settings. Your responsibilities include:
- Providing skilled nursing care according to the current nursing plan of care and within your scope of practice.
- Completing comprehensive documentation of observations, vital signs, body systems, pain, and psychosocial status.
- Performing and documenting patient wound assessments, monitoring, and care per physician orders and nursing scope of practice.
- Executing skilled nursing procedures, specimen collection, and related patient services.
- Promoting health and prevention to decrease hospitalizations through patient and caregiver education.
- Initiating and conducting education for patients and caregivers specific to care interventions and strategies identified in the nursing plan of care goals.
- Acting as a professional nursing liaison, coordinating care between patients, caregivers, physicians, and other health care providers as needed.
- Completing medication-related tasks, including administration, documentation, physician order coordination, medication change-over, and medication check-in.
Key Focus Areas of Our Person-Centered Assessments
Your role involves monitoring overall general health and providing management for:
- Chronic medical conditions (e.g., diabetes, dementia, skin ulcers, seizures, heart failure, respiratory distress).
- Minor skin injuries.
- Early identification of serious medical conditions (e.g., pulmonary aspiration, bowel obstruction, GERD, dehydration, sepsis).
- Coordination of in-home therapy services (Physical, Occupational, Speech Therapy).
- Education and support on medical status, equipment, prescribed medications, and overall health and wellness strategies.
Qualifications
- Must hold a current, valid, and unencumbered Ohio Board of Nursing License (LPN or RN).
- Current BLS (CPR) certification.
- Current and valid driver's license and car insurance.
- Ability to work independently and manage time effectively.
- Flexibility in work location and schedule availability.
- Ability to professionally communicate with a wide range of patients/caregivers and function effectively as a team player.

Salary : $2,076 - $2,442
