Case Manager

Job Purpose:
- The Case Manager at the Community Action Agency of Oklahoma will be responsible for providing comprehensive case management services to individuals and families in need. The role involves assessing client needs, developing individualized service plans, and coordinating resources to support clients in achieving self-sufficiency and improved quality of life.

Key Responsibilities:
- Conduct thorough assessments of clients’ needs, strengths, and barriers to identify appropriate services and resources.
- Develop and implement individualized service plans in collaboration with clients, ensuring alignment with their goals and needs.
- Coordinate and facilitate access to community resources, including housing, employment, healthcare, and educational services.
- Monitor and evaluate client progress, adjusting service plans as necessary to ensure effective outcomes.
- Maintain accurate and up-to-date case files and documentation in compliance with agency standards and regulations.
- Advocate for clients within the community and with service providers to ensure access to necessary resources and support.
- Collaborate with other agency staff and community partners to enhance service delivery and client outcomes.
- Participate in ongoing training and professional development to maintain knowledge of best practices in case management and community resources.
- Provide crisis intervention and support to clients as needed, ensuring their safety and well-being.
- Prepare regular reports on client progress and program outcomes for agency leadership and stakeholders.

Required Education:
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.

Required Experience:
- Minimum of 2 years of experience in case management or a related role within a community service setting.
- Proven track record of working with diverse populations and understanding of the challenges faced by low-income communities.
- Experience in developing and implementing individualized service plans.

Required Skills and Abilities:
- Strong interpersonal and communication skills, with the ability to build rapport with clients and stakeholders.
- Excellent organizational and time-management skills, with the ability to handle multiple cases concurrently.
- Proficiency in using case management software and Microsoft Office Suite.
- Ability to work independently as well as collaboratively in a team environment.
- Strong problem-solving skills and a proactive approach to addressing client needs.
- Knowledge of community resources and social service programs available in Oklahoma.
- Ability to maintain confidentiality and adhere to ethical standards in all client interactions.
