JOB PROFILE / DESCRIPTION
The QMHP is responsible for the provision of direct service care to clients and provides support and assistance to them in the community and home to stabilize current functioning. QMHP are responsible for all client related direct service documentation and reports related to the Individual Service Plans. QMHP will be responsible for submitting progress notes and discharges prior to client ending services. QMHP’s are responsible for ensuring that they receive a copy of the client's ISP to support documentations (Progress Notes). The QMHP will assist clients with support to stabilize their mental health symptoms and links to ancillary community services, and advocates for the needs of clients. The QMHP works with clients to support their ISP during the approved authorization period authorized by the client’s MCO to support clients' needs during their crisis. QMHP participate in weekly supervision/ training to improve and enhance skill level.
DUTIES AND RESPONSIBILITIES
- Provide direct mental health skill building and services to individuals undergoing crisis and requiring immediate interventions by the mental health, social services, or judicial system with the minimum age of five years.
- Provide Care Coordination and collaboration with other mental health professionals linked to clients within the community.
- Receive referral and provide telephone support for emergency crisis calls.
- Build healthy rapport and alliances with the support team and coordinate effectively with team members and Supervisor.
- Identify and utilize techniques, interventions, and strategies to enhance clients’ strengths, life and social skills, and behaviors.
- Attend all annual mandatory agency training (TOVA, CPR & First Aid, Human Rights, HIPAA, etc.) (As needed)
- Provides individual, family and supported sessions.
- Ensures supporting interventions are implemented with clients.
- Assist clients with coordinating psychiatric medication management services.
- Participates in weekly supervision.
- Implement and Revise (as needed) the Individual Service Plan for clients.
- Completes documentation that meets organization standards and maintains confidentiality.
- Reports relevant information to supervisory staff in a timely manner.
- Assist clients with ADL’s and teach them how to be self - sufficient. Provides skill building training, assisting with locating resources to assist clients in the community.
- Submit finalized Progress Notes and Time Sheets on a weekly basis.
- Responsible for submitting clinical info to the supervisor for continued stay authorizations as needed.
- Other duties and responsibilities as assigned.
JOB REQUIREMENTS
- Be available to provide services during the day, evening, weekends and holidays.
- Own an operable, registered and insured vehicle to be used for service delivery.
- Keep vehicles registered and insured during the entire length of employment.
- Provide client and guardian/family transportation in their own vehicle to conduct case management and counseling services when deemed necessary.
- Provide and maintain an acceptable driving record.
- Provide services in various neighborhoods, including crime-ridden and poverty-stricken communities.
- Conduct home visits and sessions within the homes of clients, which may present in unsanitary and/or unhealthy conditions.
- Possess, keep, use and own a personal cell phone providing the cell phone number to the Program Supervisor, Compliance Coordinator, Case Managers and Clinical Director, clients, and legal guardians for service delivery.
- Possess, keep, and check two times daily a valid CBCS email address.
- Have access to and ability to use a computer, printer, internet services, and other necessary items and equipment to produce electronic program documentation/paperwork.
KNOWLEDGE, SKILLS, AND ABILITY
- Knowledge in human services, including family and individual sessions.
- Understanding of documentation for mental health principles.
- Ability to implement an Individual Service Plan.
- Knowledge of mental health skill building activities and proficiency in implementing strategies.
- Excellent communication skills, effective writing and documentation skills.
- Knowledge of community resources and familiarity with geographical areas.
- Proficiency with computer skills and demonstrates organizational and time management skills.
- Ability to maintain a flexible schedule.
- Ability to manage multiple priorities simultaneously.
- Experience in clinical documentation and mental health.
- Experience must be with individuals who have been diagnosed with a mental illness.
EDUCATIONS AND SPECIFICATIONS
Minimum Requirement for Qualified Mental Health Professional credentials:
- A Bachelor's degree in a human services field.
- QMHP registration
- Minimum of 1 year of experience working with clients with mental health conditions.
Pay: $35.00 - $50.00 per hour
