Job Summary
The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a skilled nursing facility under the supervision of a Registered Nurse (RN) or physician. The LPN is responsible for administering medications, monitoring resident health, and ensuring high-quality, compassionate care in accordance with facility policies and Oklahoma state regulations.
AVAILABLE SHIFT: OVERNIGHT SHIFT 2200-0600 SUNDAY - THURSDAY

Key Responsibilities
- Administer medications and treatments as prescribed by a physician
- Monitor residents’ vital signs and overall health status
- Assess residents and report changes in condition to the RN or physician
- Provide wound care and other skilled nursing procedures
- Supervise and delegate tasks to Certified Nursing Assistants (CNAs)
- Maintain accurate and timely documentation of care and services provided
- Ensure compliance with infection control and safety protocols
- Assist with care planning and participate in interdisciplinary team meetings
- Educate residents and families on health conditions and care plans
- Respond to emergencies and provide appropriate interventions
- Basic computer skills for electronic medical records (EMR)
Experience
- Previous experience in a skilled nursing or long-term care setting preferred, but not required

Licensure & Requirements (Oklahoma-Specific)
- Active LPN license issued by the Oklahoma Board of Nursing
- Must pass background check and drug screening per state regulations
- Compliance with Oklahoma State Department of Health standards
Qualifications
- Current Licensed Practical Nurse (LPN) license in the state of Oklahoma
- Graduate of an accredited practical nursing program
- Current CPR certification (or willingness to obtain)
- Strong communication and interpersonal skills
