Requirements:
- Current LPN license in good standing
- Minimum of 1 year of experience in home health
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of state and federal regulations related to home health care
- Reliable transportation and a valid driver's license
Responsibilities:
- Provide skilled nursing care to patients in their homes
- Administer medications and treatments as prescribed by the physician
- Monitor and document patient progress and report any changes to the physician
- Educate patients and families on disease management, medication management, and other healthcare topics
- Collaborate with other healthcare professionals to develop and implement patient care plans
- Ensure that all patient care is delivered in accordance with state and federal regulations
