As a Home Health LPN, you will be responsible for providing quality nursing care to patients in the comfort of their own homes. You will work closely with patients, families, and other healthcare professionals to ensure that patients receive the best possible care. Requirements:

• Current LPN license in good standing
• Minimum of 1 year of experience in home health
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of state and federal regulations related to home health care
• Reliable transportation and a valid driver's license

Responsibilities:

• Provide skilled nursing care to patients in their homes
• Administer medications and treatments as prescribed by the physician
• Monitor and document patient progress and report any changes to the physician
• Educate patients and families on disease management, medication management, and other healthcare topics
• Collaborate with other healthcare professionals to develop and implement patient care plans
• Ensure that all patient care is delivered in accordance with state and federal regulations
