Under general direction, is responsible for work of moderate difficulty in performing professional case management coordination, transition services, and partnering with community service providers. Services may include providing reintegration case management and care coordination for individuals being released from incarceration. Collaborates with program team and performs related work as assigned.

The work week consists of Monday to Friday and weekends.
This position is limited appointment and grant funded with an anticipated end date of June 30, 2026.
