Responsibilities
- Evaluate patients' physical abilities, mobility, strength, balance, and functional needs.
- Develop individualized treatment plans based on each patient's condition and goals.
- Provide skilled physical therapy interventions in the patient's home.
- Help patients improve mobility, strength, balance, coordination, and independence.
- Educate patients and caregivers on therapeutic exercises, mobility techniques, safety, and home exercise programs.
- Monitor patient progress and modify treatment plans as appropriate.
- Communicate effectively with patients, families, physicians, and other members of the interdisciplinary care team.
- Complete accurate and timely documentation of evaluations, treatments, and patient progress.
- Follow professional standards, agency policies, and applicable regulatory requirements.
- Promote patient safety and encourage independence within the home environment.
- Graduate of an accredited Physical Therapy program.
- Current New York State Physical Therapist license or eligibility for licensure.
