This role is essential in ensuring a smooth and welcoming transition for new residents and their families while supporting census growth and maintaining strong referral relationships.
Key Responsibilities
- Manage the full admissions process from initial inquiry through admission
- Serve as the primary point of contact for hospitals, families, and referral sources
- Coordinate and conduct facility tours for prospective residents and families
- Review referral documentation to determine eligibility and appropriateness for admission
- Collaborate with interdisciplinary team members to ensure seamless admissions
- Maintain accurate records in EMR systems and track admissions data
- Verify insurance coverage and coordinate with billing as needed
- Follow up on pending referrals and maintain strong communication with referral partners
- Participate in marketing and outreach efforts to support census development
Qualifications
- Previous experience in admissions, healthcare marketing, or case management preferred
- Experience in a nursing home or healthcare setting strongly preferred
- Knowledge of Medicare, Medicaid, and managed care processes
- Strong interpersonal and communication skills
- Excellent organizational skills and attention to detail
- Ability to multitask and work in a fast-paced environment
