What are the responsibilities and job description for the FT Day Hospice RN Case Manager position at COASTAL HOSPICE INC?
Coastal Hospice invites you to join a team of caregivers who are committed to promoting dignity and quality of life for patients and families who face life-limiting conditions. In your role as a _RN Case Manager____, you will help ensure that every patient and family is honored, supported and comforted as they receive the right care at the right time throughout their journey by:
- Developing, implementing and evaluating a plan of patient care utilizing ANA (American Nurses Association) Standards and the Nursing Process
- Utilizing equipment and technology skillfully and is technically competent in nursing procedures
- Overseeing and/or participates in safe medication handling and practices
- Educating patients and families on significant self-care and end of life issues, all hospice services, patient liability for payment and ensures consent and payer forms are completed
- Acting as a patient advocate. Provides support, information and direct care
- Triages phone calls to identify patient/family needs and notifies Admission Services of referrals
- Overseeing and delegates assignments of CMTs, LPNs, Hospice Aides, students and volunteers on shift
- Participating in weekend, holiday, on call and/or evening/night rotations
- Maintaining appropriate licenses and adheres to continuing education obligations
- Covering different areas of hospice care to assure organizational needs are met
- Inpatient Unit RN:
- Provides medical care to hospice patients in an inpatient setting
- Completes assessments, administers medications, and manages plan of care for patients during their shift
- Coordinates discharge planning with family and medical provider
- Extended Services RN:
- Provides medical care to patients in their home environment based on triaged needs
- Utilizes nursing assessment and symptom management skills to assist with urgent after hours needs
- Educates patient/caregiver on plan of care in coordination with the on-call medical provider
RN Case Manager:
- Provides medical care to a manageable caseload of hospice patients in their home settings
- Develops plan of care alongside an Interdisciplinary Team in line with the patient/family needs and goals
- Coordinates all aspects of home hospice care including symptom management, spiritual, psychosocial, financial, and bereavement needs
- Works closely with the Interdisciplinary Team to assure patient/family goals are met
