Rapid diagnostics, advanced imaging, cath lab capabilities, and 24/7 operations enable timely, evidence-based interventions. Centralized distribution with decentralized clinical coverage places the Inpatient Clinical Pharmacist at the bedside during codes, rapid responses, and critical consults. Transitions-of-care initiatives, medication reconciliation, and discharge counseling enhance safety and reduce readmissions.
