Developmental Disabilities Professional

ACCOUNTABILITY:
Day Program Manager

DUTIES AND RESPONSIBILITES:
  • Creates and maintains a high-performance work environment characterized by positive leadership and a strong team orientation.
  • Sets the tone for person centered supports and services provided to assigned individuals.
  • Supports the activities of the program including formal and informal Person-Centered training techniques.
  • Reviews staff activities to ensure required documentation is completed accurately and submitted timely by staff for individuals on assigned caseload.
  • Performs Developmental Disabilities Professional (DDP) duties as outlined in the Department of Behavioral Health and Developmental Disabilities (DBHDD) standards and Department of Community Health (DCH) standards along with pertinent agency standards for assigned caseload.
In collaboration with the Program Manager/Director, supports the Additional Services/Enhanced Rate funding process.
  • Provides coaching and feedback on documentation submitted by assigned staff.
  • Ensures and reports changes in the condition of individual and/or any unusual occurrences related to an individual to Supervisor or Management staff in a timely manner. Reports any SIGNIFICANT changes in the condition of an individual IMMEDIATELY.
  • Ensures and reports on environmental risks/hazardous/barriers to Supervisor or Management in a timely manner. Reports any SIGNIFICANT environmental risks/hazardous/barriers to Supervisor or Management of an individual IMMEDIATELY.
  • Ensures that the health, safety and well-being of individuals are met. This may include coordination of information between Residential Program, Day Program, Nursing Staff, Behavior Specialist, Agency Leadership, Support Coordination, etc.
  • Attends ISP meetings for assigned caseload and communicates necessary information to appropriate team members.
  • Provides individual support as needed to ensure required staff-to-individual ratio is maintained.
  • Identifies training needs and materials based on Individual Support Planning meetings and/or identified needs.
  • May complete annual performance management appraisal (PMF) for assigned staff.
  • Ensures provision of and document services provided including: individualized service goals (from ISP), behavior support tracking, required notes, communication logs, and all other required documentation.
  • In the absence of the Supervisor and/or Manager, may act as the lead staff on duty for the day/determined time frame.
  • Ensures individuals are supported in personalized and age appropriate activities. This may include assisting and supporting individuals in all environments (the community and at work) so that the individual is able to learn, practice and master skills that enable him/her to enhance his/her status by assuming valued social roles.
  • Ensures individuals’ possessions are safeguarded.
  • Ensures appropriate participation and/or provide input into planning/developing implementation strategies and tracking/documentation of individual’s plan of care (to include the individual’s Support Plan and Behavior Support Plan).
  • Ensures and completes implementation strategies and documentation of Individual Support Plans (ISP)/Behavior Support Plans (BSP) at the frequency stated in the ISP/BSP. This will include resetting the documentation/ISP goals in the agency’s documentation system following completion/update on an ISP.
  • Ensures and demonstrates accurate data collection procedures and consistent use of described support strategies that promote success.
  • Ensures and documents services provided in a timely manner including: individualized service goals (from ISP), behavior support tracking, required notes and all other required documentation.
  • Ensures and completes incident reports using the online system. Ensures the report includes required information (who was involved, what happened, where did it happen, when did it happen, etc.)
  • Communicates with assigned individual’s authorized circle of support and approved external entities (i.e. Family/Substitute Decision Makers, Support Coordinators) to maintain timely and effective communication regarding individuals needs and their environment with appropriate personnel/colleagues and supervisor.
  • Ensures that all individuals are supported to explore and express their interest for community membership
  • Observe work activities and program environment to maintain (HIPAA) confidentiality requirements concerning the affairs of the individuals served, staff and the agency.
  • Completes all scheduled and required training by the announced deadline, attend and actively participate in home staff meetings.
  • Actively participate in the agency’s ongoing effort to continually improve the quality of services provided and the effectiveness of the agency’s operations by sharing ideas and insights.
  • Supports teamwork between all team members via open communication, sharing of responsibilities and by demonstrating a positive, constructive attitude.
  • Interacts professionally and effectively with supervisors/management, colleagues, families, guardians, support coordinators and authorized internal/external monitors.
  • Attends program and agency trainings as assigned/required.
  • Provides client support as needed to ensure required staff-to-individual ratio is maintained.
MINIMIM TRAINING, COMPETENCY AND EXPERIENCE:
Completion of a Bachelor’s degree and at least 1 year experience supporting individuals living with developmental disabilities.
The following professionals qualify to be a Developmental Disability Professional: Advanced Practice Nurse, Behavior Specialist, Board Certified Behavior Analyst (BCBA), Educator with a concentration in Special Education, Human Services Professional with a degree in social work or human services field, Master’s or Doctoral Degree in behavioral OR social sciences AND with specialized training, Physical or Occupational Therapist, Physician, Physician’s Assistant, Psychologist, Registered Nurse, Speech Language Pathologist or Audiologist, or Therapeutic Recreation Specialist.

Possess Georgia Driver’s License and acceptable Motor Vehicle Report (MVR). Must have basic computer skills, the ability to serve as an advocate for individuals, understand/interpret written materials, prepare incident reports and strong conflict resolution skills.
Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The job requires regular lifting and carrying up to 30 pounds unassisted, including assisting individuals when required. The job requires frequent bending, reaching, repetitive hand movements, standing, walking, squatting and sitting, with some heavy lifting, pushing and pulling exerted regularly throughout a regular work shift.

Preferred Qualifications: CPR/First Aid, CPI, Defensive Driving, ABSORB training

WORKING CONDITIONS:
Required to work 40/hours a week, Monday through Friday. General work hours will be 8:15 am - 5:00pm but are subject to change based upon programmatic needs. Work will be done in facility as well as community setting. Occasional requirements to work at other Claratel Behavioral Health facilities will also be necessary.

Review:
At least annually, incumbent will be reviewed against job duties using the agency’s Performance Management review tool. Goals may be set during this annual review and/or as the needs of the department warrant.
