Position: Registered Nurse | Location: Wheat Ridge, CO | Schedule: Monday-Friday
POSITION SUMMARY
The Registered Nurse (RN) is responsible for delivering comprehensive nursing care to residents, ensuring that care is consistent with company, federal, state, and local standards and regulations. The RN plays a crucial role in evaluating and monitoring residents’ conditions, coordinating admissions, transfers, and is charges, and ensuring the highest degree of quality care and dignity is maintained.
DUTIES AND RESPONSIBILITIES
1. Plan and deliver nursing care to residents in accordance with company standards, federal, state, and local regulations.
2. Evaluate and monitor residents' conditions, providing professional nursing care based on care plans and physician orders.
3. Administer medications as ordered by physicians and in compliance with regulations and facility policies.
4. Perform thorough assessments, identifying changes in residents' physical or psychological conditions, including lab data, vital signs, and mental status.
5. Ensure nursing histories are recorded in medical records for all residents.
6. Document nursing care provided and residents' responses accurately, timely, and descriptively in accordance with company policies and government regulations.
7. Coordinate resident admissions, transfers, and discharges, completing required documentation and forms in line with government regulations and company policies.
8. Communicate residents' conditions to families or responsible parties as requested or as changes occur.
9. Consult with physicians regarding care, treatment, and rehabilitation, ensuring timely and accurate documentation of conversations and new orders.
10. Relay information to other healthcare team members (e.g., physicians, therapists, social services) accurately and promptly.

KNOWLEDGE, SKILLS, ABILITIES & QUALIFICATIONS
1. Current and valid RN license.
2. Valid Adult Basic First Aid and CPR Certification preferred.
3. At least 1 year of experience in assisted living, home health, or long-term care industries.
4. Proficient in nursing assessments, medication administration, and documentation.
5. Ability to perform physical tasks associated with nursing duties, including bending, lifting, and moving residents with assistance.
6. Must be able to stand, walk, and perform physical activities throughout the shift.
7. Successful completion of a state approved course and is approved to administer oral and external medication.
