FIELD REGISTERED NURSE | Manhattan, NY


Key Responsibilities

  • Conduct home visits to patients
  • Complete clinical documentation and care plans
  • Coordinate care with primary care physicians
  • Update medication profiles
  • Communicate with other healthcare professionals involved in patient care
  • Maintain case notes and follow-ups using systems like HCHB
  • Participate in case conferences with supervisors


Requirements

  • Active Registered Nurse (RN) license
  • At least 6months of experience working as RN
  • Prior experience in home care or clinical nursing is preferred
  • Ability to travel to patient homes
  • Strong documentation, communication, and organizational skills
