Job Summary:
A community-focused healthcare organization is seeking a Nurse Practitioner to provide acute care services through a hybrid in-person telehealth model. This role focuses on delivering high-quality, accessible care to underserved populations in a fast-paced outpatient setting.
Key Responsibilities:
- Conduct telemedicine visits supported by an on-site clinical setting
- Assess, diagnose, and treat patients with acute care needs
- Manage a high-volume patient load (approx. 35–40 patients/day)
- Collaborate with a multidisciplinary care team
- Maintain accurate and timely documentation
Qualifications:
- Master’s degree in Nursing
- Certified Nurse Practitioner (NP)
- Minimum 1 year of urgent care experience (post-certification)
- Bilingual English/Spanish preferred
