Aging Partners’ Personal and Family Services Division is seeking a dedicated professional for a full-time, advanced social service position supporting older adults and individuals with disabilities. Responsibilities include providing information and referral services, assisting with benefits, and empowering individuals to make informed, self-directed decisions. As an ADRC Options Counselor, this position will develop action plans with individuals and their families or caregivers, conduct follow-up contacts to evaluate the effectiveness of those plans, and determine whether additional support is needed. The role ensures that each individual receives accurate, comprehensive, and up-to-date information and assistance.
This position is in-office setting with availability for public interaction over the phone and in-person, 8:00 - 4:30 Monday - Friday.MINIMUM QUALIFICATIONS: Graduation from an accredited four-year college or university with major coursework in social work, gerontology, psychology, human services, or a specified field of study to fulfill state-mandated qualifications plus two years of experience in providing program services or case management to a geriatric or disabled population including six months of supervisory experience; or any equivalent combination of training and experience that provides the desirable knowledge, abilities, and skills that meet the state mandated requirements.
NECESSARY SPECIAL REQUIREMENTS:
Possession of a valid driver’s license when operating a vehicle is necessary to the satisfactory performance of assigned duties.
