About the Role
We’re seeking a compassionate and driven Physical Therapist Assistant (PTA) to join our therapy-owned home health agency. In this role, you’ll work closely with Physical Therapists to deliver personalized, high-quality care to patients in their homes, helping them regain mobility, reduce pain, and return to independence.

What You’ll Do
- Carry out individualized treatment plans under the direction of a Physical Therapist
- Lead patients through therapeutic exercises and mobility training
- Apply modalities such as ultrasound and electrical stimulation
- Monitor progress and communicate updates to the supervising PT
- Educate patients and families on home exercises and safety strategies
- Maintain a safe and organized care environment
What We’re Looking For
- Graduate of an accredited PTA program
- Current state PTA license (required)
- Home health experience preferred
- AHA CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
- HomeCare HomeBase experience is a plus
Physical Demands
Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
Must be able to lift, carry, pull, or push up to 100 lbs..
