What You’ll Do
- Perform start-of-care visits and ongoing patient assessments
- Develop and manage individualized care plans
- Provide hands-on nursing care, education, and emotional support
- Monitor patient progress and adjust care as needed
- Collaborate with physicians and interdisciplinary teams
- Supervise LVNs/LPNs and Home Health Aides
- Ensure accurate, timely documentation (Homecare Homebase preferred)
- Build strong relationships with patients and families
What You’ll Need
- Active RN license
- Valid driver’s license, auto insurance & reliable transportation
- CPR certification
- 2 years of RN experience (home health preferred)
- Strong clinical judgment and communication skills
