What You’ll Do
- Build and grow strong referral relationships across hospitals, SNFs, ALFs, and physician offices
- Position Choice Hospice as the provider of choice through education and trust
- Identify new opportunities and expand your territory
- Collaborate with clinical teams to ensure seamless patient transitions
- Represent the organization at community events and networking opportunities
- Meet and exceed monthly admission goals
Who You Are
- Background in healthcare (sales, clinical, or referral coordination)
- Skilled at building relationships and influencing decisions
- Driven, self-motivated, and goal-oriented
- Comfortable working in the field and managing your own territory
- Hospice or home health experience is a plus—but not required for the right person who has healthcare background
Requirements
Minimum of 2 years home health or hospice sales experience preferred
Valid Driver’s License
Valid Auto Insurance
HomeCare HomeBase experience preferred
