We are seeking a compassionate and motivated PRN Speech Language Pathologist (SLP) to join our growing Home Health team. This flexible role is ideal for clinicians who value autonomy, meaningful one-on-one patient care, and the ability to truly impact recovery in a home-based setting.
As a Home Health SLP, you will provide skilled therapy services to patients in their homes, helping them regain communication abilities, improve swallowing safety, and enhance overall quality of life.
What You’ll Do
- Evaluate and treat patients with speech, language, cognitive-communication, voice, and swallowing disorders in the home setting
- Develop individualized care plans based on comprehensive assessments and physician collaboration
- Provide dysphagia management, including swallowing strategies and safe diet recommendations
- Implement therapy techniques to improve articulation, fluency, cognition, and functional communication
- Train patients and caregivers on communication strategies and safe swallowing techniques
- Document evaluations, progress notes, and discharge summaries in compliance with regulatory and agency standards
- Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care
- Adjust treatment plans based on patient progress and home environment needs
What We’re Looking For
- Current Speech Language Pathologist (SLP) license in the state of Nevada
- Certificate of Clinical Competence (CCC-SLP)
- Previous home health experience strongly preferred, but not required
- Strong clinical judgment and ability to work independently
- Excellent communication and patient education skills
- Reliable transportation for in-home visits
Requirements
Valid Speech Language Pathologist License
1 year of rehab experience preferred, not required
CPR certification
Valid Driver’s License
Valid Auto Insurance
Long Term Care experience preferred
Vital-Stim certification preferred
HomeCare HomeBase experience preferred
Physical Demands
Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
Must be able to lift, carry, pull, or push up to 100 lbs..
