As a Physical Therapist, you will play a critical role in helping patients recover from injuries, manage chronic conditions, and improve their overall physical function and mobility. Your expertise will be essential in designing and implementing personalized rehabilitation programs that address each patient's unique needs, focusing on orthopedic and geriatric populations as well as sports-related injuries. You will work closely with patients to restore movement, reduce pain, and prevent disability through evidence-based manual therapy techniques and therapeutic exercises. As a Physical Therapist, you will:
Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Applies concepts of infection control/universal precautions in performing patient care
Assure continuity of quality patient care delivered with appropriate documentation
Provide effective communication to patient/family, team members and other health care professionals
Promote agency philosophy and administrative policies
Maintain and complete all documentation within company stated guidelines.
Requirements
Successful completion of accredited physical therapy program and state licensure exam.
Valid Physical Therapy license
1 year of home health experience preferred
CPR certification
Valid Driver’s license
Valid Auto Insurance
HomeCare HomeBase experience preferred
