We are seeking a compassionate and motivated Occupational Therapist (OT) to join our growing home health team. In this role, you’ll have the opportunity to deliver meaningful, one-on-one care that helps patients restore independence, improve function, and return to daily life with confidence.

What You’ll Do
- Conduct comprehensive clinical evaluations to determine patient needs and therapy appropriateness
- Develop, implement, and update individualized Plans of Care
- Provide skilled OT interventions to improve:
- Strength, coordination, and range of motion
- Functional mobility and safety
- ADL performance (self-care, dressing, bathing, eating, etc.)
- Assess cognitive, sensory-motor, psychosocial, and environmental factors affecting function
- Fabricate and maintain splints and adaptive equipment
- Recommend home/environmental modifications to promote safety and independence
- Participate in interdisciplinary care planning (IDG) to ensure coordinated patient outcomes
- Maintain accurate, timely clinical documentation (evaluations, progress notes, orders)
- Follow infection control and safety standards in all patient care activities
What We’re Looking For
- Current Occupational Therapist (OT) license in good standing
- Strong clinical judgment and patient assessment skills
- Ability to work independently in a home health setting
- Excellent communication and teamwork skills
- Compassionate, patient-centered approach to care
- Experience in home health is a plus, but not required

Requirements
Valid Occupational Therapy License
1 year of occupational therapy experience preferred
CPR certification
Valid Driver’s License & Auto Insurance
HomeCare HomeBase experience preferred
Physical Demands
Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
Must be able to lift, carry, pull, or push up to 100 lbs..
