PRN Position Summary
The Home Health Social Worker LMSW PRN plays a critical role in delivering compassionate and comprehensive social work services to patients receiving home health care. This position focuses on assessing patients' psychosocial needs, coordinating care plans, and providing emotional support to patients and their families in their home environments. The role requires collaboration with interdisciplinary teams to ensure holistic care that addresses both medical and social determinants of health. The social worker will advocate for patient rights and access to community resources, facilitating smooth transitions between care settings. Ultimately, this position aims to enhance patient well-being, promote independence, and improve quality of life through tailored social work interventions.
Minimum Qualifications:
As a Social Worker, you will:
Assesses and completes the Comprehensive Assessment within 5 days of the patient’s election of hospice care to identify patient and family’s psycho social, financial, environmental and community needs as evidenced by POC documentation, clinical records, IDG meetings and community resource referrals.
Assists with advance directives, funeral planning, discharge planning, family counseling and may assist in changes in the level of care as indicated by the patient or family’s needs.
Use effective interpersonal relations and communication skills to achieve desirable outcomes.
Provides patient and family/caregiver counseling around issues of end-of-life, death, and grief.
Attends interdisciplinary team meetings, participates in the patient care planning process, and coordinates and collaborates with the IDG to promote coordination of patient care.
Provides an environment that promotes respect for patients and their privacy and property.
Requirements
Master's degree in Social Work from an accredited school
Licensed Master Social Worker
One or more years of Hospice experience preferred
Valid Driver’s License
Valid Auto Insurance
AHA CPR Certification
HomeCare HomeBase experience preferred
