What You’ll Do
- Complete start-of-care, recertification, and routine skilled nursing visits
- Perform comprehensive patient assessments in the home
- Develop and update plans of care based on patient needs
- Educate patients and families on disease management and self-care
- Monitor condition changes and communicate with physicians/team
- Supervise LVNs/LPNs and Home Health Aides as needed
- Document visits accurately and timely
- Ensure compliance with home health regulations and payer requirements
What You Bring
- Active RN license
- Valid driver’s license & auto insurance
- CPR certification
- Home Health experience strongly preferred
- Experience with HomeCare HomeBase is a plus
