Job Summary:
The Hispanic Case Manager at Chippewa Valley Free Clinic serves as a vital bridge between the clinic and the Hispanic/Latino community, ensuring that vulnerable patients receive compassionate, culturally competent care. This part-time role combines case management, translation, and community outreach to connect eligible patients with the health services they need. Duties/Responsibilities:
· Serve as the dedicated Case Manager for the Hispanic/Latino patient population
· Coordinate with the Director of Clinical Services to address language and culture barriers in patient care
· Work with navigators to move patients through the clinic effectively and efficiently
· Educate patients on available services, eligibility, and the importance of self-care and follow-up
· Provide Spanish translation and interpretation between medical professionals and patients
· Help patients fill out English-language forms, schedule appointments, and understand medical information
· Work with CVFC staff to develop patient messaging materials
· Enroll patients in the Wisconsin Well Woman's Program and assist with related paperwork
· Connect patients to community resources, outside agencies, and transportation when necessary
· Ensure referrals and follow-up have occurred across all services
· Make reminder calls for appointments to reduce no-show rates
· Co-facilitate the Hispanic Initiative committee with the volunteer chair
· Document accurate patient information in charts and EMR
· Prepare accurate and timely reports for grants and board meetings
· Other duties as requested
Clinic Support: Maintain a consistent and reliable presence during clinic operations in accordance with the established schedule, providing direct support to ensure efficient and compassionate patient care.
Supervisory Responsibilities:
Direct supervisor of Interpreters (volunteers).
Required Skills/Abilities:
· Case management experience preferred
· Fluent in Spanish and English; proficient in translation, preferably in the medical field
· Knowledge of medical terminology in both languages
· Strong verbal and written communication skills
· Ability to work independently and within a team structure
· Strong organizational skills with the ability to prioritize
· Working knowledge of various software and database technologies, including Microsoft Office and EMR
· Demonstrates respect for diversity
· Awareness of psychosocial barriers that impact the lives of the uninsured
· Embraces and supports the mission of the Chippewa Valley Free Clinic
Education and Experience:
· High School Diploma or equivalent
· Highly proficient in both English and target language vocabularies typically used in formal, consultative, and casual modes of communication, including colloquial slang, idiosyncratic slang, and regionalisms.
· Knowledgeable of specialized medical vocabulary (terminology) in both English and Spanish.
Work Environment & Physical Demands
· Clinical/medical office setting with moderate noise levels
· Potential exposure to infectious diseases and biohazard materials
· Frequent bending, standing, walking, and active listening throughout the clinic
· Ability to lift to 15 lbs.
· Regular interaction with patients experiencing illness, emotional distress, or language barriers
