What are the responsibilities and job description for the Case Manager position at Children's Resource Center?
Case management services include, but not limited to:
o Coordinate assessments and treatment planning; participate in the development of an individualized service plan (ISP) with participating of client/family, as well as other service providers
o Maintain professional collateral contacts with other service providers (i.e. schools, JFS, Juvenile Court, etc.) as needed with proper authorizations from client/family and supervisor
o Provide training and parental guidance to advocate for and facilitate linkages in the use of basic community resources to assist the client/family to:
· Achieve treatment goals
· Maximize independence
· Increase social support networks (i.e. relatives, friends, community organizations) to assist them in reduction of life stressors
· When applicable, transport client/family to appointments such as medical, mental health, and/or housing
o Provide assistance to clients in crisis situations
o Facilitate home-based services as a significant part of service delivery within client’s natural environments
o Provide face-to-face contact with families (through individual, family, and/or group treatment modalities as appropriate) to provide support in increasing parenting skills such as behavioral intervention, understanding child development issues, special needs, and communication
o Possess specialized knowledge and skills to understand growth and development in children and adolescents to apply appropriate range of needed interventions
Documentation/Caseload Management
o Maintain up to date treatment plans, Diagnostic Updates, and progress notes
o Complete progress notes through concurrent documentation (this includes any and all contact with client and family)
o Manage assigned caseload
o Maintain client service hours as determined by agency standards
Development and coordination of Youth and Young Adults in Transition Program
o Maintain program and service outcomes
o Conduct needs assessment activities, develop resource inventory, create program materials as requested by program manager
o Community outreach to increase knowledge of YYAT program
o Coordinate with internal providers to increase awareness and understanding of program and activities
Attend regularly scheduled supervision, team meetings, and general staffing meetings
Adhere to and uphold policies and procedures as established by the CRC
Maintain current licensure as required by agency at point of employment
Establish professional goals, participate in trainings, conferences, workshops, etc. to remain current on intervention practices and techniques
Ability to work in a fast paced, highly collaborative environment
Consultations with community members regarding crisis and suicide prevention
Conduct oneself in a professional manner
