Our aim is to protect the health and safety of our clients by partnering with community resources as part of a continuum of care to meet the individual and family's medical, educational, and employment needs.
PRIMARY JOB FUNCTION
The Floating Senior Case Manager is directly responsible for the delivery of Case Management services to clients living at CRF’s Programs servicing families or single adults. The Senior Case Manager will work at different shelter locations to ensure continuity of Case Management services to clients at programs with gaps in Case Management staff.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- The Senior Case Manager will be responsible in providing Case Management Services to the clients during their program assignment.
- Senior Case Manager must be able to utilize DHS CARES system to complete all required forms, assessments, and case notes timely and accurately; review and update client information; as well as review accessible client information from Human Resource Administration, Administration for Children Services, Department of Probation, Department of Homeless Services, etc. to inform the service planning for the clients and ensure coordination of services.
- Responsible in maintaining the client chart current and updated with all case notes and copies of required documentation.
- Assess and identify the clients' needs, formulate appropriate plans of service, monitor and document progress of service plans and document all significant interactions with or on behalf of the clients to ensure the clear, effective delivery of service to families and individuals and the provision of high quality social services that are in compliance with the State, City and Agency regulations
- Assist clients in accessing entitlements, public assistance, childcare, education, healthcare, mental and behavioral health services, employment related services, and housing resources
- Collaborate with outreach teams to ensure coordination and consistency in service delivery of services to the clients
- Provide advocacy and referral services to ensure clients obtain all required documentation to receive appropriate benefits, entitlements and services. Advocates on behalf of clients to ensure clients receive all benefits, entitlements and services
- Collaborate with the interdisciplinary team to ensure a client-centered and holistic approach to addressing client’s barriers to services and overall needs in a timely fashion to ensure compliance with State, City, and Agency regulations
- Provide appropriate individual counseling and crisis intervention services to families and individuals to help stabilize and minimize the negative impact of the family's or individual’s displacement
- Conduct unit/room inspections as required by the Department of Homeless Services to ensure that individuals and families with children are living in a healthy and safe environment. Promoting crib safety and safe sleep practices for families with infants aged 0- 12 months old
- Conduct wellness checks and monitors of high-risk clients are monitored to ensure that any issues discovered during the wellness check are immediately addressed
- Lead workshops and groups and participates in meetings as requested
- Prepares case summaries and must have knowledge of client issues and concerns and must be an active participant in case conferences and interdisciplinary team meetings.
- Must be knowledgeable of interagency service providers to ensure clients receive the appropriate medical, psychiatric, etc., services needed within the community
- Ensure adherence to CRF and DHS policies, reporting requirements, and confidentiality standards (including HIPAA)
- Prepares and submits data and reports as required by CRF
- Contributes to the overall success of the organization by performing all other duties and responsibilities as assigned by supervisor or designee, and being able to pivot with ever-changing priorities and newly added job-related responsibilities that arise based on business changes, needs, and demands
MINIMUM QUALIFICATIONS
- Bachelor’s degree in social work or a related discipline such as Sociology, Psychology, Counseling, Criminal Justice, or Human Services is required
- At least 4 years of experience in social services, counseling, crisis intervention, mental health/substance use treatment, family services, preventive services, or housing services are desirable
- Demonstrated knowledge of and proficiency in CARES
- Demonstrated experience working in shelter setting with families and single adults.
- Knowledge and ability to work effectively with a wide range of community-based providers
- Working knowledge of Case Management theories and approaches
- Knowledge of DHS/OTDA social service documentation policies and procedures
- Experience facilitating groups for homeless adults, specialized knowledge of substance abuse issues and treatment, and family dynamics treatment experience is required
- Bilingual language ability is highly desirable.

- Work Schedule: May be required to work long hours and a varied schedule to meet job demands and deadlines. Therefore, this job role requires someone to work evenings and weekends, which will be determined by the needs of the specific program.
- Periodically may bend, lift, and carry objects weighing up to seventy-five pounds.
