Shift: Full-time Monday through Friday from 8:30 am to 5:00 pm. There is also an expectation to work 1-2 weekend days a month and flex off a day during the week.

Summary

The Community Health Worker Team Leader is responsible for providing leadership to the front-line staff in collaboration with direct leadership. Serves as a conduit between the front-line Community Health Worker (CHW) employees and leaders for the purpose of facilitating communication, overseeing orientation of new team members, and evaluating professional practice with a particular focus on quality and performance improvement. Responsible for the results and development of assigned team members. Plans and executes day-to-day operational needs by setting priorities, planning for and addressing staffing needs, and coordinating work across a team(s) as needed. Provides guidance, coaching, and feedback to team members and provides significant input to employment decisions, addresses performance issues and administers performance reviews. Monitors key metrics and analyzes information to solve on-going problems to improve team performance. The Team Leader will spend 70% of their time in working shifts and 30% of their time in administrative/leadership, recognizing that at least 10% of their time in the clinical shifts is still spent coaching, mentoring, and developing staff. Specific responsibilities and areas of focus vary based on the department or program in which the Team Lead is assigned.

Responsibilities

  • People Leadership:
  • Provides people leadership for assigned staff, including hiring, onboarding, scheduling oversight, performance management, and professional development
  • Conducts recurring staff rounding to provide real-time support, coaching, feedback, and recognition
  • Administers performance evaluations and corrective action in partnership with leadership and Human Resources
  • Promotes a positive, inclusive work environment grounded in MODEL Leadership and Just Culture principles
  • Ensures staff are informed of relevant changes within the unit, department, and organization
  • Supports staff accountability for meeting patient care standards related to safety, effectiveness, patient rights, and service excellence
  • Uses employee engagement feedback and survey results to develop and implement action plans for continuous improvement
  • Operational Leadership:
  • Oversees staff scheduling and coverage, including new hire orientation schedules and appropriate skill mix
  • Manages personnel processes, including PTO approval, attendance monitoring, and leave tracking (e.g., FMLA, STD, LOA)
  • Monitors staffing trends and addresses dependability concerns as needed
  • Ensures accurate timekeeping and payroll readiness by reviewing timecards and addressing discrepancies
  • Maintains effective working relationships with providers and interdisciplinary partners
  • Regulatory & Quality Compliance:
  • Ensures regulatory readiness and compliance with applicable policies, standards, and requirements
  • Monitors staff licensure and certification status to ensure ongoing compliance
  • Ensures timely submission of required employee documentation to Human Resources
  • Maintains compliance with privacy and confidentiality requirements, including protection of Patient Health Information (PHI)
  • Promotes adherence to infection prevention and safety practices in accordance with organizational guidelines
  • Miscellaneous:
  • Participates in leadership meetings, committees, and assigned projects to support departmental and organizational priorities
  • Ensures staff provide education and facilitate learning for patients, families, and care teams in a culturally responsive manner that respects diversity and inclusion
  • Collaborates with physicians, families, and interdisciplinary teams to support development and implementation of appropriate plans of care
  • Advocates for patients, families, and staff by identifying concerns and supporting resolution of ethical or clinical issues
  • Maintains knowledge of healthcare, community, and system resources to support effective navigation and coordination of care across settings

Work Experience

How You’ll Be Successful:

  • At least 3 years related experience Required
  • Leadership experience Preferred

EDUCATION

  • High school diploma or equivalent Required
  • Four-year Bachelor's degree or equivalent experience Preferred

LICENSES AND CERTIFICATIONS

  • Current Community Health Worker/Promotor license in the State of Texas Required
  • Current Community Health Worker/Promotor Instructor license in the State of Texas Preferred
