Role Description
The Registered Nurse Care Coordinator will serve as an integral part of the care team, providing coordinated, patient-centered care. Daily responsibilities include assessing patient needs, creating and implementing care plans, communicating with families, physicians, and other healthcare professionals, and ensuring continuity of care. Additional duties involve supporting patient education and assisting families in navigating healthcare systems and resources. This is a full-time, on-site position located in Dallas, TX.
Qualifications
- Active Registered Nurse (RN) license in the state of Texas
- Proficiency in care coordination, patient assessment, and planning individualized treatment paths
- Strong communication and interpersonal skills for interacting with families and healthcare teams
- Ability to manage cases, navigate healthcare systems, and access community resources to support patient care
- Knowledge of pediatric health-related care and experience in family-centered approaches
- Detail-oriented with strong organizational and time-management skills
- Bachelor of Science in Nursing (BSN) preferred, or equivalent relevant experience
- Prior experience in pediatric care or care coordination is strongly preferred
