Case Manager Job Responsibilities:
- Providing ongoing case management; coordinating behavioral health services, medical care, education, placement, finances, and socialization services to children and families with intense/complex needs.
- Collaborating with the family and team members to create support plans that accurately capture the strengths, needs and culture of the family.
- Facilitating Child and Family Team meetings; establishing an agenda, setting and maintaining a strength based meeting environment, staying within timeframes, giving all team participants the opportunity to contribute, holding people accountable for their commitments and the ability to assist a team in developing a consensus for children and families.
- Assisting young people and families in securing needed community resources and linkages to other services. Helping families to identify natural and informal supports.
- Service delivery can be inclusive of skills training and family support.
- Interacting professionally with other stakeholders: Division of Child Safety (DCS), Division of Developmental Disabilities (DDD), juvenile probation, Direct Service Providers (DSP), etc.
- Performing other duties related to the position
Case Manager Education/Experience Needed:
- High school diploma or GED.
- Must be BHT with two (2) years of related behavioral health work experience, or a related bachelor’s degree (preferred but not required).
- Must be 21 years or older
- Valid Arizona Driver's License with an acceptable 39 month driving record
- Reliable transportation, including proof of Insurance
- Candidates are required to submit a negative drug test
- Obtain an Arizona DPS Level One Fingerprint Clearance Card
- Complete Adult, Child, and Infant CPR and First Aid training
