Position: Case Manager
Status: Full-Time
Responsibilities:
- Conduct intake orientation with all patients, explaining rules, policies and procedures
- Complete Case Management assessments and reports
- Develop working relationships with outside agencies
- Evaluate patient needs and providing referrals as appropriate.
- Liaison between referral sources and provide updates on patient progress in treatment
- Assist patient in obtaining necessary resources (housing, food stamps, medical and benefit assistance)
